Med spa software, compared
Most med spas need two jobs done: a practice system that handles booking, charting and checkout, and a growth system that answers every lead and follows up until they book. Zenoti, Boulevard, Vagaro and Mangomint are built for the first job. GoHighLevel, which we build on, is built for the second. Many clinics keep their practice system and add GoHighLevel on top.

What follow-up automations changed, our team's data
Missed calls turned into text conversations with missed-call text-back (81 clinics)
Calls answered after adding an AI receptionist (58 clinics)
Show rate after 72 h, 24 h and 2 h reminders confirmed by reply (77 clinics)
Disclosure
We are a med spa marketing team that builds on GoHighLevel. We don't resell Zenoti, Boulevard, Vagaro or Mangomint, and this page has no affiliate links. Everything we say about those tools comes from their own websites and help docs, linked at the bottom of the page. Where a vendor doesn't describe a feature, we say "check with the vendor" instead of guessing. We left out prices because they change often. Get a current quote from each vendor.
The problem
Practice management software is good at the patient who has already booked. The money leaks earlier, between the ad click or phone call and the first appointment. That's the part most buyer's guides skip.
Across 186,400 leads in our team's data, people who got a reply in under a minute booked at 41%. After 4 to 24 hours it was 9%.
Only 21% of missed callers called back in our team's data. A booking calendar can't fix a call nobody answered.
Before automation, 23% of leads at the clinics our team worked with were never contacted at all.
The comparison
We compared each tool on what matters for growth, based on what each vendor publishes about itself. Features change and often depend on the plan, so confirm anything important in a demo.
Sources: each vendor's own website and help docs, checked October 29, 2026. Links are listed further down this page. Aesthetic Record and PatientNow also show up on many shortlists as clinic-first EMRs. The same split applies: check what they do for lead follow-up before you assume it's covered.
Who each one suits
Built for groups. It lists memberships that work across locations, cross-location reporting, e-prescribing through Surescripts, and its own AI receptionist and missed-call texting. Its pages focus on the clinic side; how deep its lead follow-up goes is worth testing in a demo.
A strong front desk for single and multi-location clinics. It lists scheduling around rooms and devices, smart charting with medical sign-off, and the Beau AI receptionist. Its med spa page doesn't describe missed-call text-back or instant replies to ad leads.
Broad booking, POS hardware, memberships with autopay and SOAP notes. Its AI receptionist, Vera, replies to messages but doesn't answer phone calls.
Booking, forms and charting on a HIPAA-compliant plan, plus Mira, which handles calls and texts. Check which plan includes charting, and how it connects to other tools.
A marketing and follow-up platform. It handles the steps between lead and booking: instant text and call, missed-call text-back, Voice AI, nurture and reminders. It is not an EMR, and HIPAA is a paid add-on you have to switch on.
If your practice system already works, keep it. Use it for charting, checkout and memberships. Add GoHighLevel on top for lead response and follow-up, connected through the vendor's API or Zapier where your plan allows.
Our data
This is the part a software demo won't show you. Each row is one automation our team built in GoHighLevel, measured at the median clinic before and after.

Some practice systems offer pieces of this, like reminders or an AI receptionist. The question to ask in every demo is whether the tool runs the whole chain: answer in seconds, follow up for two weeks, confirm the visit, recover the no-show.
Methodology: before = a clinic's first 90 days, after = months 4 to 6. Figures are medians across clinics, January 2024 to June 2026. Full method and limitations are in our med spa speed-to-lead study.
How to choose
Pick one system of record for charts, consents and photos. Everything else connects to it.
Ask every vendor that will touch patient data, including your marketing tools. Get the answer in writing.
Call the demo number after hours. See if you get a text, an AI answer or voicemail.
Submit a test form from a Facebook or Google lead ad. Time the first text and the first call.
One reminder isn't nurture. Ask to see a two-week sequence for a lead who hasn't booked.
Ask whether your plan includes API access, webhooks or Zapier, and whether bookings sync both ways.
How we build it
We test your phones, forms and ads, time your replies and map what your practice system already automates.
Your practice system keeps charts and checkout. GoHighLevel takes the leads, calls and texts, with bookings synced where your plan allows.
Instant replies, missed-call text-back, Voice AI, nurture, reminders and no-show recovery. You get a monthly report on booked consults.
Where it fits
Once you know which system does what, the build matters more than the logo. Our med spa CRM page shows the pipeline, workflows and reactivation we set up in GoHighLevel. For calls, see how an AI receptionist for med spas books after hours.
Two parts of the follow-up have their own guides: how to reduce med spa no-shows with reminders and card on file, and med spa email marketing for nurture.
As a med spa marketing agency, we run the ads and the follow-up together, on top of whatever practice system you already use.

FAQ
Med spa software runs a medical spa's daily work: booking, charting and consents, payments, memberships and reminders. Some clinics add a separate marketing platform, such as GoHighLevel, to answer new leads fast and follow up until they book.
It depends on your setup. Zenoti lists features for groups, like memberships that work across locations, cross-location reporting and e-prescribing. Boulevard lists scheduling around rooms and devices, smart charting with medical sign-off and an AI receptionist. Demo both with your own workflows.
Usually not. GoHighLevel isn't a clinical charting system or a front-desk clinic POS. Many clinics keep their practice system for charts and checkout and use GoHighLevel for lead response, follow-up and reminders.
Not by default. HighLevel offers HIPAA as a paid, account-wide add-on that includes a signed Business Associate Agreement, and it must then be turned on for each sub-account. Confirm the current terms with HighLevel and your compliance advisor.
Software prices change and depend on locations and add-ons, so get a current quote from each vendor. For our setup and follow-up work, we share exact pricing after a free audit.
